How do I obtain an Indiana Juvenile or Adult Criminal History Record?

Juvenile History Record: In order to obtain a Juvenile History Record, you will need to go your local police department and be fingerprinted.  You will then mail the fingerprints and a money order for $10, (Paid  to the Indiana State Police), To: The Indiana State Police, 100 N Senate Avenue, IGCN, Room N340, Indianapolis in 46204, Attn: Anita Rice.  Do not fold or crease the fingerprints.  The juvenile criminal history will then be mail to you.  If you have additional questions regarding contact Anita (ISP) @ (317)234-2631.
